TMJ & TMD Specialist Care in Sydney

At Designer Smiles, we diagnose and manage TMJ (TMD) disorders that affect the jaw joint, chewing muscles, and bite (occlusion).

Your first TMJ appointment includes a thorough history, clinical examination (jaw range of motion, muscle/joint palpation, bite evaluation), and – only when it changes treatment decisions – TMJ imaging such as X-rays or CBCT (for bone/joint detail) and MRI via referral (for disc and soft tissue assessment).

From there, we create a clear, step-by-step TMJ treatment plan that prioritises conservative care and measurable milestones.

Care is led by prosthodontist Dr Sarkis Nalbandian and coordinated with physiotherapy. We focus on TMJ pain relief, function, and prevention, escalating treatment only when indicated.

Is TMJ Treatment Right for Me?

You may benefit from a TMJ specialist assessment if you have:

  • TMJ symptoms such as jaw or facial pain, tenderness around the temples or in front of the ears
  • Clicking, popping, or grating on opening and chewing
  • Headaches, jaw stiffness, or limited opening and locking
  • Ear-adjacent pain or pressure or tooth sensitivity not explained by dental disease
  • Night-time clenching or grinding (bruxism), morning jaw fatigue, or visible tooth wear
  • A history of jaw trauma, arthritis, or orthodontic bite concerns

If symptoms affect sleep or daily function, or have recently escalated, our Sydney CBD clinic can clarify the cause and next best steps.

Benefits of Seeing a Specialist for TMJ Disorder

  • Clear diagnosis and plan: Which structures are involved (muscle, joint, disc, bite) and what to do first
  • Conservative first: Most patients improve without surgery; we measure progress at each review
  • Reduced pain and better function: Targeted care for chewing, opening, and comfort
  • Multidisciplinary support: Dental care coordinated with physiotherapy and, when needed, medical input
  • Education and prevention: Self-care, habit coaching, and protection from further tooth wear
  • Transparent options: We explain TMJ splint vs night guard, expected timelines, and costs

How We Treat TMJ Dysfunction at Designer Smiles

We tailor your plan to your diagnosis. A typical pathway:

Conservative Care (usually 2–6 weeks)

  • TMJ self-care: Soft diet during flare-ups, jaw rest, heat and ice as directed, awareness of daytime clenching
  • Physio-guided exercises and stretches: Gentle range-of-motion, relaxation and posture strategies
  • Short-term medication (if appropriate): Anti-inflammatories or muscle relaxants under clinician guidance
  • Start with 1–2 safe TMJ exercises;

Oral appliance Therapy (8–12 weeks average)

  • Custom TMJ splint therapy (e.g., stabilisation splint) to reduce muscle overactivity, protect teeth from bruxism, and support the bite
  • Follow-ups at 2, 6, and 12 weeks to review comfort, wear patterns, and symptom change
  • We’ll explain splints and mouthguards so you understand the differences and indications

Adjunctive Therapies (case-by-case)

  • Trigger-point therapy or botulinum toxin for refractory muscle hyperactivity
  • Bite rehabilitation or orthodontic collaboration when occlusion is a significant driver and symptoms persist
  • Guidance for TMJ arthritis and myofascial pain management

Imaging and referrals (when indicated)

  • X-rays and/or CBCT for joint and bony evaluation
  • MRI referral when disc displacement or internal derangement is suspected
  • Surgical referral pathways (arthrocentesis/arthroscopy/orthognathic) are reserved for specific diagnoses after conservative care

What to Expect at Your First Appointment With Our TMJ Specialists

  • Symptom review and medical/dental history
  • Jaw movement and muscle/joint examination
  • Bite/occlusion assessment
  • Discussion of likely diagnosis, triggers, and immediate self-care
  • Whether imaging is recommended now or later
  • Your personalised plan, timelines, and review schedule

Our approach: Start simple, measure response, and escalate only when the evidence (and your goals) support it.

Frequently Asked Questions (FAQs)

What is the best treatment for TMJ pain?

Most patients improve with conservative care: self-care, TMJ exercises, and a custom TMJ splint when indicated. We individualise your plan to target the main drivers (muscle vs joint vs bite).

Do I need a TMJ splint or a night guard?

Both protect teeth, but a stabilisation splint is designed for TMJ muscle/joint support, while a generic night guard mainly protects from wear. We’ll assess your symptoms and explain the right choice for you.

How much does a TMJ splint cost in Sydney?

Most custom TMJ mouth splints fall within a typical range that reflects materials, customisation, and review visits. We’ll outline options and inclusions at your consultation.

When are CBCT or MRI scans needed for TMJ?

Not everyone needs imaging. CBCT helps assess bone/joint structure; MRI shows the disc and soft tissues. We recommend scans when the results will change your treatment plan.

Can TMJ cause ear pain, dizziness, or tooth sensitivity?

These can occur with TMJ-related muscle and joint dysfunction, but other conditions can mimic them. We’ll examine you, to rule out other dental or ENT causes and guide appropriate care.

Are TMJ exercises helpful, and which are safe to start?

A gentle range of motion and relaxation drills can reduce TMJ flare-ups. We’ll teach 1–2 safe TMJ stretches at your first visit.

Is TMJ serious or permanent?

Many cases are manageable with conservative care. Some chronic conditions require ongoing habit and splint support; we’ll set realistic timelines and goals.

How long does TMJ last?

Many patients notice improvement within 2–6 weeks of conservative care. Splint therapy typically runs 8–12 weeks with reviews and adjustments.

How long do TMJ flare-ups last?

Most muscle-related TMJ flares ease within 24 to 72 hours with rest, a soft diet, and gentle jaw exercises. Joint-driven flares can take 3 to 14 days, and longer-lasting or recurrent symptoms usually mean an underlying factor (disc, arthritis, bite, or habits) needs targeted treatment. If your pain persists beyond two weeks, if your jaw locks, or if your bite is changing, book a TMJ assessment.

Dr Sarkis is the maestro of cosmetic dentistry….for years I suffered with 2 slightly darker front teeth due to a childhood accident. I’ve had composite veneers before but nothing compared to the composite veneers Dr Sarkis did……now I feel so confident smiling again……Dr Sarkis I can’t thank you enough…

Monica McKenzie

Quite literally the best dentist I’ve been to in a lifetime. Dr Natasha Koelmeyer specifically is just a wonderfully kind and gentle Doctor. Must also mention that the head specialist here is also second to none. Their knowledge (goodness they know their stuff) their approach, their kind and gentle manner - my daughter actually enjoys going to the dentist. They’ve got a really considered patient centric mentality. I wouldn’t go anywhere else, every single person there including the reception staff is a star. Thank you Designer Smiles!

My journey with Dr Sarkis began about 10 years ago. I did not realise that I had lacked self confidence and shone away from "my true smile". Dr Sarkis was honest, reassuring and professional throughout my process and not only was he able to achieve that perfect smile but achieve same without compromising the health of my teeth and gums. The ongoing care at Designer Smiles is 5 star whether it is a routine check, clean minor or major dentistry. Thank you Dr Sarkis and also your team.

Christina Vergotis

I found Designer Smiles from one of their YouTube videos showcasing the Icon treatment to fix white spots on teeth. Dr. Sarkis Nalbandian's consultation provided me with a thorough plan and showed me how their practise could give me predicable results for my concerns. I knew I was in safe hands. I had Kristy Gupta perform both of my procedures and she was very professional and so kind. I am very happy with my results!

Winsome Kain

I cannot express how grateful I am for Sarkis and the team at Designer Smiles. Around 11 years ago my life changed when Sarkis first transformed my smile with composite veneers. This month, with my original teeth in tact I returned to have my composite veneers upgraded to porcelain. I was amazed that Sarkis and his team could do this without an injections, no pain and whilst still keeping my original teeth in tact. I absolutely love the result and I would highly recommend that anyone that is unhappy with their smile in any way, must visit designer smiles

Matt Williams

Started my treatment with the team at Designer Smiles in 2019. Had other dental treatments including Maxillofacial, expanders, straightening and gum grafting but out of all the procedures and surgeries Dr. Sarkis was the key as he was applying the ‘finishing product’ which included composite veneers. I am gobsmacked by the incredible job done. He talks the talk and definitely walks the walk. And his support team there are simply incredible and make you feel part of a family not just another dental clinic to take your money. Can not recommend highly enough. Customer for life here!!
